Fee for Service Speech Language Pathologist Per Diem

Position Summary

The New Jewish Home's Certified Home Health Agency (CHHA) is seeking dedicated and experienced Per Diem Speech-Language Pathologists (SLPs) to join our Community Services team.

In this role, the Speech-Language Pathologist will provide home-based speech therapy services to adult and geriatric patients throughout Manhattan. Responsibilities include conducting comprehensive speech, language, cognitive-communication, and swallowing assessments, developing individualized treatment plans, and providing therapeutic interventions designed to maximize patients' communication abilities, swallowing function, safety, and overall quality of life.

• →Conduct speech-language and swallowing screenings, evaluations, and assessments as prescribed by the physician.
• →Develop and implement individualized treatment plans to address speech, language, voice, cognitive-communication, and swallowing disorders.
• →Provide therapeutic interventions and rehabilitative services based on each patient's clinical needs and functional goals.
• →Educate patients and family members regarding treatment plans, progress, home exercise programs, and discharge recommendations.
• →Prepare and maintain accurate clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries.
• →Monitor patient progress and modify treatment plans as appropriate.
• →Collaborate with physicians, nurses, therapists, social workers, and other members of the interdisciplinary care team to ensure coordinated, patient-centered care.
• →Participate in case conferences, team meetings, and care planning discussions.
• →Maintain required departmental and regulatory records in a timely and accurate manner.
• →Assist in maintaining therapy equipment, supplies, and work areas.
• →Participate in professional development and educational activities to remain current with best practices and advancements in the field.
• →Perform other duties as assigned.
