• →Offer advice, information, and support to clients, including referrals for aftercare focused on maintaining an abstinent lifestyle.
• →Assist Therapists with risk management plans and reviews.
• →Ensure clients follow program rules, supporting the manager in investigating breaches, including substance testing.
• →Conduct intakes, assessments, and discharges efficiently.
• →Keep accurate client records and maintain databases.
• →Facilitate transfers between agencies and services for clients.
• →Support clients with medication reminders, conduct audits, and maintain accurate records.
• →Work with clients to create recovery plans that are realistic and achievable.
• →Help clients attend external appointments in line with their care plans, providing transport support or accompanying them as needed.
• →Assist with day-to-day operations, helping clients manage practical aspects of daily living such as life skills, housing, and benefits.
