Responsibilities

→Establishes achievable short- and long-term goals for patients
→Implements nursing protocols based on the treatment plan, ensuring continuity of care for patient. Provides education to patient and caregiver(s)
→Evaluation of the patient’s status and efficiency of the nursing care and modified care plan as needed
→Helps provide healthcare access through nurse visits, management of in-basket task (phone messages, MyChart communication, refill requests, results)
→Provides safe and timely telephonic triage for symptomatic callers, using system approved triage policies and protocols
→Coordinates surgery scheduling processes for patients
→Other responsibilities as assigned

→Current Minnesota RN Licensure and knowledge of specialty area
→Associate’s (ADN) or Bachelor’s Degree in Nursing (BSN)
