Local Consultant-Development of Midwifery Investment Case-Dar Es Salaam-Tanzania

UNFPA will support MoH to develop a Tanzania specific Midwifery Investment Case. This case will guide strategic investments, advocate for policy formulation, and resource mobilization, with the ultimate goal of improving maternal and neonatal health outcomes in Tanzania. 
 

The consultant will work under the overall guidance and supervision of the Assistant  Representative.

• Generation rigorous evidence, country-specific investment case for advocating of institutionalization of midwifery models of care in Tanzania.
• Develop actionable recommendations to ensure institutionalization of midwifery models of care, quality assured maternal health commodities and innovative financing mechanisms, to secure predictable, equitable, and sustainable implementation of midwifery models of care.
• Conduct impact modeling to project outcomes including health impacts (such as maternal and neonatal lives saved and morbidity reductions under different scale-up scenarios),costs, workforce analysis, financing requirements, and potential economic and social returns.
• Develop and tailor advocacy messages and materials for SMART advocacy to ensure institutionalization of Midwifery models of care that translate social-economic evidence into actionable insights for diverse policy and decision-maker audiences.

The consultant will be responsible for the following tasks:

 Inception and Stakeholder Engagement

• Organize and facilitate an inception workshop with Government of Tanzania officials, UN agencies, midwifery associations, CSOs, NGOs, academia, and development partners.
• Develop a stakeholder engagement plan to ensure broad participation and buy-in.

 Situation Analysis.

• Conduct a detailed review of Tanzania’s maternal and neonatal landscape analysis, health system, policies, and current midwifery services and workforce analysis, and fiscal space for health and SRMNCAH.
• Identify gaps in coverage, financing, training, and workforce distribution.
• Define Midwifery Intervention Packages: In line with the investment case framework and national priorities and select a package of evidence-based interventions that can be delivered by midwives in the Tanzania context.

Modeling Health Impacts, Costs, Workforce Requirements and Economic Benefits of Scaling Midwife-delivered Interventions.

• Perform comprehensive analysis guided by the UNFPA Investment case framework for scaling up care provided by midwives
• Utilize the Lives Saved Tool and other relevant tools to project health outcomes of different scale-up scenarios following institutionalization of midwifery models of care, including maternal and neonatal lives saved, and reductions in maternal and neonatal  morbidity, and other relevant outcomes, as well as costs and workforce needs
• Develop multiple investment scenarios in consultation with government officials reflecting different levels of intervention coverage and resource allocation to institutionalize midwifery models of care.
• Estimate the subsequent economic benefits of improved health through scaling midwifery models of care.

Workforce Analysis and Policy Development

• Estimate the required number of midwifery workforce for  institutionalization of midwifery models of care. 
• Conduct a gap analysis of existing workforce capacity, skills, and distribution for institutionalization of  midwifery models of care.
• Provide suggestions on a feasible strategy and estimate the cost of meeting the identified workforce gap (e.g. by scaling up pre-service training, improving deployment of midwives..)

Enabling Environment and Policy Analysis

• Assess systemic, regulatory, and policy factors affecting the institutionalization and scale-up of Midwifery Model of Care.
• Provide recommendations for improving the enabling environment, including professional recognition, regulation, financing and integration into health systems.

 Validation and Dissemination

• Prepare and facilitate validation workshops and stakeholder consultations.
• Incorporate feedback into the final investment case document.
• Support the dissemination of findings through policy briefs, advocacy materials, and high-level events.

Key Deliverables

The following key outputs are expected:

• Inception Report: Detailing methodology, work plan, stakeholder engagement strategy, and data sources.
• Situation Analysis Report: Overview of maternal and neonatal health landscape analysis, and midwifery services.
• Draft Investment Case Report: Including impact projections, workforce strategies, and policy recommendations.
• Final Investment Case Document: Fully validated and ready for dissemination, including advocacy materials.
• Final Policy Brief: Fully validated and ready for dissemination, summarizing the key results and messages of the investment case
• Workshop Reports: Documentation of validation and stakeholder engagement sessions.

The consultancy will be conducted for 60 days, commencing from the date of contract signature. The sixty days consultancy will be structured into distinct phases: an initial inception workshop and planning phase, followed by data collection, review, and analysis. 
Subsequently, the focus will shift to drafting reports and stakeholder validation, culminating in the finalization, dissemination, and submission of all deliverables.
 

A clear milestone-based payment is outlined for this consultancy, with each payment tranche released upon the successful submission and approval of specific deliverables.
The payment is made upon receiving a confirmation email for the final approval from UNFPA (and MOH if needed) on the deliverables.

Payment Schedule
Payments will be made into three tranches based on the milestones and deliverables met by the consultant upon successful submission and approval from UNFPA Tanzania Country Office.

The consultant will report to the UNFPA Tanzania Country Office Assistant Representative. Technical guidance will be provided by designated UNFPA technical team members based at the country office, regional and Head Quarter offices. Regular review meetings will be held to assess progress and resolve challenges. Existing national coordination platforms will be leveraged, particularly the MoH’s line Directorate of Human Resource Development and Division of Nursing and Midwifery Services (DNMS) for technical input and oversight as well as RMNCAH TWG, to ensure alignment with ongoing initiatives, policies, and priorities for Tanzania's Health Sector.

Dar es Salaam, Tanzania - Remote Work

The consultant is required to attend any meetings specifically requested by the UNFPA country office. Travel costs for activities outside Dar es salaam will be covered by UNFPA, according to the established rules and guidelines. This includes transportation and per diem for missions related to data collection, stakeholder consultations, and co-facilitation of the national Inception workshops as outlined in the Terms of Reference (TORs).

These specific missions will be determined in advance and coordinated with the designated UNFPA project team.

 Master’s Degree in Health Economics, Public Health, Health Financing, or related health sciences.

 Knowledge and Experience: 

• Minimum 5 years of professional experience in health financing, investment case development, or cost-benefit analysis, preferably in reproductive health.
• Experience with standard costing tools (SPECTRUM/FamPlan, LiST, WHO Choice, or equivalent)  or demonstrated experience with impact modeling tools relevant to SRMNCAH service packages.
• Demonstrated experience in engaging with Government, UN agencies, and development partners.
• Excellent analytical, facilitation, and communication skills in English.
