RESPONSIBILITIES:
The primary purpose of this position is to support patients receiving care in Aspire Rural Health System's Emergency Departments and inpatient settings by identifying and addressing social determinants of health that may affect their ability to access healthcare, manage chronic conditions, obtain medications, and safely transition back into the community following discharge.
The Community Health Care Worker collaborates with Emergency Department staff, inpatient care teams, social workers, care managers, and community partners to connect patients with appropriate resources and services. Responsibilities include conducting patient outreach and follow-up calls, assisting with Medicaid applications and benefit enrollment, coordinating referrals to community resources, and following up on identified patient needs after discharge.
This position provides support across multiple Aspire locations and may require travel between facilities. The Community Health Care Worker plays an important role in reducing barriers to care, improving care transitions, encouraging appropriate follow-up with primary care providers, and supporting efforts to reduce avoidable Emergency Department visits and hospital readmissions.
