Responsibilities

As a Community DSP, you’ll play a vital role in supporting individuals to reach their personal goals while enjoying independence and inclusion in their community. You will:

→Follow each person’s support plan to help them work toward their personal goals
→Help with everyday tasks like personal care, meals, and daily routines when needed
→Make sure the people we support are safe, healthy, and comfortable at all times
→Drive and go along with individuals to appointments, activities, and outings in the community
→Be a positive role model and speak up for the people you support when needed
→Help with light cleaning and household tasks to keep the home running smoothly
→Take part in team meetings, trainings, and agency events
→Write brief notes about your shift so the next staff and supervisors know how things went

High School Diploma or equivalent
Valid NYS Driver’s License
