This position is 24 hours a week, part time for Diabetes education and pump education!

Responsibilities

→Educates and counsels on health-related topics such as diet/nutrition, exercise, stress and chronic disease management.
→Assesses the nutritional status of patients using parameters such as anthropometric measurements, visual/physical assessment and interpretation of lab values with documentation in the medical record and/or patient forms.
→Assesses and creates care plans for patients with diabetes and obesity, including but not limited to inpatient diabetes and cardiac interventions, diabetes self-management education and support classes.
→Assists in maintaining programming accreditation [ex: Diabetes Self-Management Education and Support (DSME/S) and Diabetes Prevention Program] including required trainings and education requirements.
→Participates and assists in planning of community relations events, health fairs and presentations.
→Maintains a working knowledge of all program-related core competencies.

Requirements

Certification or Licensure:

Registered Nurse, Registered Dietician or Pharmacist.
Certified Diabetes Educator (CDCES), or Board Certified in Advanced Diabetes Management (BC-ADM) preferred
Pump Trainer Certification or willingness to obtain

Education:

Bachelor’s degree required.

Experience:

At least 1 year working with a diabetic patient population.

Skills:

Good computer skills; excellent customer service and communication skills.
Bilingual (English/Spanish) preferred.

Entry salary dependent upon education, skill set, and experience.
Salary range: $37.50-$56.23 / hour
