Visiting Angels of Fairfield has an immediate need for a full-time Client Service Coordinator to supervise in-home care to seniors within our community. Visiting Angels is a non-medical home care agency, delivering compassionate, high-quality companion and personal care to seniors in their homes or places of residence. We are seeking candidates with a positive, can-do attitude and capable of working in a fast-paced work environment.

POSITION PURPOSE:  
The Client Service Coordinator is responsible for determining and managing our clients’ total care needs. This individual will work with new clients to assess daily needs and develop a customized care plan. The CSC will also develop a weekly care schedule and identify a suitable caregiver. Candidates should have experience working with seniors and exhibit excellent verbal and written communication skills, the ability to problem solve, and work on multiple tasks simultaneously.


PRINCIPAL ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS:
The primary responsibilities are described below but do not represent a comprehensive list for this position. Additional tasks may be assigned periodically as necessitated by business demands.
  • Process all referrals received from intake, direct email or phone in a timely manner
  • Match clients with appropriate caregivers who can fully meet the needs required
  • Ensure all cases are staffed appropriately
  • Ensuring staff arrives on time and arrange coverage for late and called out/no show caregivers
  • Foster relationships with clients and family members
  • Provide exceptional support to both clients and caregivers by responding to issues and requests in a timely, professional manner
  • Obtain, document, and maintain all pertinent information with respect to clients and caregivers in Wellsky system 
  • Communicate client concerns and caregiver issues to appropriate team members without delay 
  • Assist in coordinating referrals to community agencies, when appropriate 
  • Must be available to be on-call (paid) for evenings and weekends on a rotating schedule
REQUIRED JOB KNOWLEDGE AND SKILLS:
  • College degree
  • Experience working with seniors in their home or in a facility
  • Ability to multi-task
  • Able to work in a fast-paced environment and manage disruptions
  • Exceptional problem-solving skills
  • Exceptional written and communication skills
  • Ability to use discretion, prioritize and make decisions independently and in concert with other staff 
  • Ability to resolve client and caregiver issues in an efficient and timely manner
  • Strong interpersonal skills and the ability to work well in a team environment
