Our interdisciplinary team works closely with participants, caregivers, healthcare providers, and community partners to ensure each individual receives compassionate, personalized care while remaining active and connected within their community.
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Adult Day Health team. The LPN will provide direct nursing care and clinical support to participants, helping promote their health, safety, independence, and overall quality of life.
Working closely with the RN and interdisciplinary Care Team, the LPN will implement individualized care plans, administer medications and treatments, monitor changes in participants' health and functioning, and provide appropriate clinical support. This position plays an important role in helping participants remain healthy and engaged in the community while reducing unnecessary hospitalizations and re-hospitalizations.
Responsibilities
~1 min read- →Provide direct nursing care in accordance with individualized care plans, physician orders, organizational policies, and the Massachusetts LPN scope of practice.
- →Monitor participants for changes in physical, cognitive, behavioral, and functional status and communicate significant findings appropriately.
- →Administer medications and treatments as ordered and monitor participants for effectiveness, adverse reactions, and other concerns.
- →Collaborate with the RN and interdisciplinary Care Team to coordinate and implement participant care.
- →Communicate with families, caregivers, physicians, nursing facilities, residential providers, and other community-based providers as directed.
- →Maintain accurate, timely, and thorough clinical documentation in accordance with organizational policies and regulatory requirements.
- →Provide health education and clinical training to participants, caregivers, and staff within the LPN scope of practice.
- →Provide clinical guidance and support to program staff related to participant care.
- →Monitor prescribed dietary, nutritional, and feeding requirements outlined in participant care plans.
- →Follow all applicable nursing, infection prevention, safety, emergency, regulatory, and organizational policies and procedures.
- →Respond appropriately to changes in participant condition and initiate interventions and notifications within the LPN scope of practice.

Requirements
~1 min read- Current, unrestricted Massachusetts Licensed Practical Nurse (LPN) license.
- Knowledge of applicable nursing standards, regulations, and clinical practices.
- Strong communication, organization, time-management, and problem-solving skills.
- Ability to prioritize responsibilities and work effectively both independently and as part of an interdisciplinary team.
- Fluency in English.
Nice to Have
~1 min read- 1–2 years of nursing experience working with older adults or individuals with chronic health conditions, memory loss, or dementia.
- Previous Adult Day Health experience.
- Fluency in Cape Verdean Creole or another additional language.
The LPN works in a clean, well-lit Adult Day Health environment with fluctuating temperatures and close interaction with participants and staff.
The position requires:
- Frequent sitting and substantial computer use, including keyboard and mouse operation.
- Regular walking and standing throughout the workday.
- Ability to negotiate stairs.
- Ability to lift, carry, push, and pull equipment and supplies weighing up to 35 pounds.
- Adequate visual acuity and manual dexterity to safely operate clinical equipment.
- Ability to perform repetitive tasks while maintaining attention to detail and accuracy.
