Case Manager- Chambers Park

The Case Manager provides direct support services to veterans residing at Chambers Park, an independent living apartment complex serving veterans in Marion, Indiana, to promote housing stability, self-sufficiency, and successful community integration. This position requires an individual who can work independently, demonstrate strong organizational and communication skills, and have experience working in social services and with vulnerable populations.

• →Provide direct support and case management services to veterans residing at Chambers Park.
• →Assess veteran needs and connect individuals to community resources that promote independence and long-term housing stability.
• →Assist veterans in developing goals that support self-sufficiency and successful community integration.
• →Coordinate and facilitate activities that promote engagement, wellness, and community connection.
• →Document client interactions, services provided, and veteran progress in accordance with program requirements.
• →Assist veterans with transportation needs to access appointments, services, and community resources.
• →Work collaboratively with an interdisciplinary team to ensure coordinated and comprehensive care.
• →Maintain professional relationships with community partners and resource providers.
• →Perform duties independently while demonstrating sound judgment, organization, and effective communication skills.

  Applicants must have an associate's degree in social services or human services with relevant experience or a Bachelor's degree.  A valid drivers license is required.
