  • Join us to support a multidisciplinary clinical team across Northern Tasmania
  • Shape high-quality, person-centred mental health care through Connected Care
  • SCHADS 3B ($40.49-$43.42p/h) + Superannuation, depending on qualifications and experience

Connected Care is seeking an experienced and passionate Peer Worker to draw on their lived experience to build authentic connections and support participants in their recovery

This is an opportunity to bring together your lived experience, personal growth and specialist peer work knowledge and skills. You will work alongside our participants to build authentic connections and support meaningful outcomes.

Connected Care is a statewide, integrated mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania (PHT).

Supporting adults aged 18 years and over, Connected Care provides free mental health care and support through a ‘no wrong door’ approach and stepped-care model. Our centralised statewide after-hours service provides additional access to support outside core service hours.

In this role, you will support our peer workforce across the Connect Care, strengthening peer practice and capability, fostering a inclusive team culture, and ensuring recovery-oriented and lived experience-informed approaches across service delivery.

This is a field-based role, delivering outreach support across Tasmania to participant and their families. You will support participants to understand and exercise their rights, strengthen self-advocacy and identity barriers to choice and autonomy and active participation in their own life, care and support.

In this role, you will:

  • Develop mutually respectful relationships that create opportunities for connection, shared learning, growth and new possibilities
  • Use lived experience intentionally and purposefully to foster trust, reciprocity, learning and meaningful connection
  • Support participants to explore identity, wellbeing, purpose and meaning
  • Facilitate connections to community, culture, family, relationships, valued roles and natural supports that strengthen belonging, wellbeing and healing
  • Adapt peer support approaches in partnership with participants to reflect their strengths, preferences, goals, relationships and aspirations.
  • Certificate IV in Mental Health, Mental Health Peer Work, or Community Services (completed or enrolled), or equivalent lived experience workforce knowledge, skills and practical experience
  • Personal lived experience of mental health challenges, distress and/or engagement with mental health service
  • Demonstrated understanding of peer work values and principles
  • A strong understanding of recovery-oriented and trauma-informed practice
  • Right to work in Australia
  • A current Driver Licence and willingness to travel as required
  • Satisfactory National Police Record Check
  • International Police Check (if applicable)
  • Current Working with Vulnerable People/Working with Children Check and NDIS Worker Screening Check
  • NDIS Worker Orientation Module Certificate – Quality, Safety and You – free online course
