Performs patient assessment and plans, delivers and evaluates patient education to selected populations across the continuums of illness and lifespan. Assumes a role as a resource person by participating in community outreach and education support.

1. Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) through the Commission of Dietetic Registration (CDR)

2. Must possess current licensure as required by state board where services will be provided:

1. Certified Diabetes Care and Education Specialist (CDCES) Certification.

1. Two (2) year of clinical experience with One (1) year in the care of diabetic patients.

1. Five (5) year of clinical experience with One (1) year in the care of diabetic patients.

Responsibilities

1. Skill in organizational matters, including time management, prioritization, multitasking, and problem-solving.

2. Ability to manage multiple priorities in a fast-paced environment and drive results.

3. Ability to use excellent customer service, written and oral communication skills.

4. Basic knowledge of computer programs such as work processing, spreadsheet, and email applications.

5. Knowledge of relevant scientific principles, established standards of care and/or research findings.

6. Knowledge of procedures and techniques involved in administering routine and special treatments to patients.
