The Supervisor Community Health collaborates with the Manager of Preventive Medicine Department and Preventive Medicine Nurse Navigator to supervise Community Healthcare Workers' scheduling, payroll, community programs, and preventive medicine assignments, and data collection. Assists in monitoring performance improvement and hospital wide requirements for employees, patient care and safety.

2. Valid Driver’s License.

3. CPR Certification.

1. Some vocation or college courses completed.

1. Three (3) years of experience as a Community Healthcare Worker or in a healthcare setting.

To be completed as established by department: (Administered at Department level)

1. Community Healthcare Workers or Community Health Education Resources Person Level 2

1. Experience with Electronic Health Records.
