Responsibilities

→Full-time position with a flexible schedule based on family needs
→Regular home and community-based visits required
→Evening and weekend availability may be necessary
→On-call availability to respond to emergencies as needed
→Travel throughout the community is required.

→Manage a caseload of up to 10 families
→Complete assessments and required documentation within established timelines
→Develop service plans and goals in collaboration with clients and families
→Conduct regular home visits and provide ongoing support services
→Assist families in accessing community resources and supportive services
→Maintain relationships with referral sources, schools, service providers, and community partners
→Prepare court reports and testify in court when required
→Participate in supervision, team meetings, and agency trainings
→Ensure compliance with DCFS, agency, and confidentiality requirements

Requirements

Bachelor's degree in Social Work, Psychology, Human Services, or a related field required
Experience working with youth and families preferred
Knowledge of DCFS regulations and child welfare services preferred
Bilingual English/Spanish preferred
Valid Illinois driver's license, reliable transportation, and proof of insurance required
Ability to work effectively with diverse populations and community partners.
