• →Conduct comprehensive patient assessments to evaluate their healthcare needs.
• →Develop, implement, and update individualized care plans based on patient assessments and preferences.
• →Administer medications and treatments as prescribed by the physician.
• →Monitor and document patient progress and responses to treatment.
• →Educate patients and their families on health management, medication administration, and care plans.
• →Coordinate care with interdisciplinary team members to promote optimal health outcomes.
• →Ensure compliance with agency policies, state, and federal regulations.
• →Act as a patient advocate, promoting their rights and preferences in care.

• Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
• Graduate of an accredited nursing school.
• At least 1 year of nursing experience; experience in home health preferred.
• Reliable vehicle and valid driver's license.
• Eligible to work in the United States.
