Case Manager

Anew is an outpatient behavioral health provider that helps individuals facing mental health and substance use disorders.

• →Coordinate and provide care athat is safe, timely, effective, efficient, and client-centered.

• →Manage case assignments, draft service plans, and review case progress regularly.

• →Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.

• →Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.

• →Facilitate clients in achieving personal independence and assist in developing daily living skills.

• →Perform other duties as assigned.

• →Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates.

This position does not supervise any staff.

• Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.

• Knowledge of behavioral health practices and community resources.

• Ability to work independently and as part of a team.

• High school diploma or equivalent is required; 

• a college degree in Human Services is preferred.

• CPR certification is preferred.

• NPI number/Ohio Medicaid ID is required (instructions on how to apply will be provided).

• Minimum of 3 years in behavioral health or a related field is preferred, but not mandatory.

• Requires sitting for extended periods and occasional lifting of up to 20 pounds.

• Must pass all Federal and State background checks according to company policy.
