Social Worker - Leaving Care Team

Liquid Personnel is recruiting Social Worker for its client's Leaving Care Team.

This is an excellent opportunity to support young people as they transition into adulthood, helping them achieve positive outcomes and develop the skills needed for independent living.

As part of a dedicated and supportive team, you will work with care-experienced young people aged 16-25, providing guidance, assessment, and pathway planning to ensure they receive the support they need to thrive. • →Manage a caseload of young people transitioning from care to independence.
• →Undertake assessments and develop Pathway Plans in line with statutory requirements.
• →Work directly with young people to identify their needs and aspirations around education, employment, training, accommodation, health, and wellbeing.
• →Build positive relationships with young people, advocating on their behalf where required.
• →Liaise with partner agencies and professionals to coordinate effective support and services.
• →Ensure statutory visits, reviews, and recording are completed within required timescales.
• →Maintain accurate and up-to-date case records and reports in line with professional standards and legislation.

To be successful in this role you must have, 

• Social Work England registration
• Eligibility to work in the UK
• A full UK driving licence
• Post-qualified experience within Children's Services
• Degree level qualification in Social Work (or equivalent)
• Experience working with Looked After Children, Leaving Care, or Care Leavers teams is desirable
