As a Member Navigator (Care Manager), you are at the heart of the Avenues experience, serving as a trusted advisor, advocate, and lifelong partner to our members. You don’t just support individuals; you build meaningful, enduring relationships that empower people to age with confidence, dignity, and independence.
Avenues is a membership-based program designed for individuals who wish to age confidently and safely in their own homes. As a Member Navigator, you’ll be part of a forward-thinking team dedicated to elevating the standard of aging well and you’ll play a pivotal role in making that vision a reality.
