Job Title: Registered Nurse (RN) Case Manager – Home Health | Reclaim Your Work-Life Balance Company: Home Health Care, Inc. Locally owned and trusted throughout Minnesota for more than 30 years. What You'll Do Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall. Complete OASIS assessments and individualized plans of care. Provide skilled nursing visits, assessments, and patient education. Coordinate care with physicians and the interdisciplinary team. Help patients remain safe and independent in their homes. Maintain accurate and timely clinical documentation. What We're Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong clinical judgment and ability to work independently. Excellent organization and communication skills. Reliable transportation for travel throughout the territory.
