Responsibilities
~1 min read- →Engage with eligible patients in person at partner clinics and introduce Medsien’s Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.
- →Educate patients on the benefits of CCM and RPM services and answer questions about the programs.
- →Guide eligible patients through the enrollment process and ensure all required information and documentation are completed accurately.
- →Communicate clearly and compassionately with patients and their families throughout the enrollment process.
- →Coordinate with clinic staff and Medsien’s care management team to support a smooth patient enrollment experience.
- →Maintain accurate records of patient interactions and enrollment status.
- →Work toward established patient enrollment goals.
- →Build positive relationships with patients and clinic staff while representing Medsien professionally.
Requirements
~1 min read- Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification required.
- 1+ years of experience working in a clinical setting, preferably in a care management or chronic disease management role.
- Strong communication skills and the ability to work collaboratively with clinical staff and patients.
- Proficiency in medical terminology and clinical processes.
- Must be highly organized, detail-oriented, and capable of handling multiple tasks in a fast-paced environment.
- Willingness to travel to different clinic locations in the Trenton or Detroit, MI area as needed.
