21st Century Home Health Services (21HHS) is the fastest-growing home health agency in Northern California. We care for patients across San Francisco, San Mateo, Santa Clara, Santa Cruz, Alameda, Contra Costa, Solano, Napa, Yolo, Placer, El Dorado, Sacramento, Marin, and Sonoma counties.

There's a reason for that growth. Words like collaboration, teamwork, and productivity stay buzzwords unless the structure, model, and tools make them real. At 21HHS, we've built a model that combines the best of home health with the structure, resources, and support clinicians need to thrive. Our clinical guiding principles are simple: empower, enhance, educate.

When clinicians get real support and structure, the results show up in two places at once: patient outcomes and employee satisfaction. We know our clinicians and therapists provide outstanding care because our patients, their families, and their caregivers tell us so. Check out our Yelp and Google reviews. We're proud of this work.

We've also set a new benchmark for employee satisfaction in home health. As a Top Workplace, 21HHS supports growth and recognition through open communication and professional development. Feedback from our team on Glassdoor and Indeed backs this up. Our outcomes, low turnover, rapid growth, and high morale speak for themselves.

We give you great tools and resources, and a lot of them, plus the support you need to succeed and enjoy your work. In return, we ask that you use those tools to do your job well. When the right mindset meets the right resources, the payoff is real: high job satisfaction, excellent patient outcomes, and earning potential that's hard to match. That's the 21HHS Standard.

Please note: All opportunities at 21HHS require being in the field, visiting patients in their homes.

In this role, you will initially be responsible for follow-up visits under the guidance of an experienced Case Manager RN, so you're never on your own while you learn our way of doing things. As your confidence grows, you'll take on more comprehensive visits and get hands-on exposure to case management — a natural next step in your career.

  • Anticipated coverage area: Sacramento Area*
  • Schedule options: Tuesday–Saturday, Sunday–Thursday, or Wednesday–Sunday. Weekend coverage will be focused on primarily on Admissions, Start of Cares
  • Employment Status: Full Time (5 days per week)
  • Productivity Expectation: Average 35+ visits per week
  • Support: A dedicated Case Manager RN, weekly case conferences, and AI-powered charting to cut down on paperwork
  • Growth: A clear path toward comprehensive visits and case management

*You'll be assigned a primary territory, but may be asked to cover outside it for vacancies, leaves of absence, or increased census. 21st Century Home Health Services makes every effort to keep you in your primary area; any reassignment is temporary and supports safe patient care.

  • Average 35+ visits per week by making the most of our resources and tools — staying independent, productive, hardworking, organized, and sharp in your critical thinking
  • Provide hands-on, skilled nursing care that follows the plan of care set by your Case Manager RN and our clinical guiding principles — Educate, Enhance, Empower — teaching patients and families about their diagnosis and care plan, and helping them build self-care skills that last
  • Prepare equipment and supplies for skilled procedures, and use resources thoughtfully
  • Keep your Case Manager RN up to date, and report important clinical changes to them and the patient's provider promptly
  • Spot when a patient could benefit from home health aide, PT, OT, speech therapy, or social work support, and flag it to your Case Manager
  • Join weekly case conferences to share updates and coordinate care
  • Partner with your Case Manager on Start of Care assessments, documentation, and care plans as needed
  • Pitch in on weekend on-call coverage as part of a shared rotation
  • Complete clear, accurate visit notes within 24 hours — with help from AI-powered charting tools
  • Document patient needs, goals, limitations, responses to care, and any incidents or changes in condition
  • Protect patient privacy in everything you do
  • Build your clinical skills and take an active role in your own professional development
  • Use what you learn about your patients to prioritize your day and meet the visit goals set with your Case Manager

You'll work with a wide range of patients and procedures, including:

  • PICC lines, IV antibiotics, and TPN administration
  • Wound care and wound vacs
  • Kangaroo and feeding pumps; NG, G, and J tube management
  • Drains (JP, biliary, nephrostomy) and PleurX drainage
  • Foley and suprapubic catheters; ostomy management
  • Trach care
  • Blood draws and SQ/IM injections
  • Staple and suture removal; compression wraps
  • 1+ year of nursing experience (RN or LVN).
  • Associate's, Bachelor's, or Master's degree in Nursing
  • Current, unencumbered California RN license
  • Current Basic Life Support (BLS) card
  • Valid California driver's license and an acceptable driving record
  • Comfort with technology and electronic documentation
  • Strong critical thinking, communication, and organizational skills, and the ability to manage your own day
  • 1 year of home health experience
  • Experience with high-acuity patients
  • Experience on an interdisciplinary team

All licenses and certifications must be current when you start and kept up to date, along with annual compliance training. As part of our vetting process, you'll complete a computer literacy assessment and a personality assessment — we want to make sure you can navigate comfortably in our environment. You may also be asked to help with other duties as assigned by your Case Manager RN.

* You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs. While 21st Century Home Health makes every effort to maintain your primary territory, temporary reassignments may be necessary to ensure safe, high-quality patient care and to meet the needs of our growing organization.
