Occupational Therapist – Hospital Discharge

Liquid Personnel is recruiting a Locum Occupational Therapist for its client to join Adult Social Care Hospital Discharge team based in London. 

You will provide specialist occupational therapy assessments and interventions to support safe, timely and effective hospital discharges under the Care Act 2014 and the Discharge to Assess (D2A) model.

The role focuses on assessing functional abilities, identifying risks and arranging appropriate equipment, adaptations and rehabilitation support to help residents return to the least restrictive environment and maximise their independence.

You will also contribute to reducing hospital delays, preventing readmissions and improving outcomes through effective multidisciplinary working. 

• →Recognised Occupational Therapy qualification.
• →Current HCPC registration.
• →Experience undertaking occupational therapy assessments with adults.
• →Knowledge of the Care Act 2014 and strengths-based practice.
• →Understanding of hospital discharge processes and the Discharge to Assess model.
• →Experience assessing functional ability, moving and handling needs, equipment and adaptations.
• →Strong communication, assessment, prioritisation and multidisciplinary working skills.

Working Pattern: Hybrid, with flexibility to work remotely and from the office
