ComForCare Home Health Care - Brantford is looking for a compassionate and dedicated Personal Support Worker (PSW) to join our growing team in BRANTFORD, ON. This is a rewarding opportunity to make a real difference in the lives of clients by providing essential care and support right in the comfort of their own homes. Responsibilities:

Provide personal care assistance including bathing, grooming, dressing, and hygiene support
Assist clients with mobility, transfers, and ambulation to ensure safety and comfort
Prepare nutritious meals and assist with feeding as required
Perform light housekeeping duties including laundry, cleaning, and tidying
Monitor and document client health status and report changes to the care team
Offer companionship and emotional support to clients and their families
Assist with medication reminders and adherence to care plans

Requirements:

Valid Personal Support Worker (PSW) certificate from an accredited Ontario program
Previous experience in home care, long-term care, or a related healthcare setting preferred
Current CPR and First Aid certification
Reliable transportation and valid driver's license to travel to client homes in Burford and surrounding areas
Strong communication skills and a compassionate, patient-centered approach
Ability to work independently as well as collaboratively with a care team
Vulnerable Sector Check (VSC) required or willingness to obtain
