Senior Social Worker – Community Geriatric Service

Are you an experienced Social Worker seeking an opportunity to work at the forefront of community-based geriatric care? As our service continues to expand, we are looking for a passionate and skilled clinician to join our highly supportive multidisciplinary team.

This is a rewarding and professionally stimulating role for an experienced case manager who thrives in complex care environments and is committed to helping older people remain safely and independently in their own homes.

About the role:

As a Senior Social Worker within the Community Geriatrics team, you will play a pivotal role in delivering person-centred care to older adults with complex medical, functional and psychosocial needs. Working within a case management model, you will provide advanced clinical assessment, care coordination and therapeutic intervention in a community setting.

Key responsibilities include:

Conducting comprehensive assessments using a range of formal and informal assessment tools.
Developing, coordinating and reviewing tailored care plans that address complex client needs.
Facilitating access to appropriate community, health and support services.
Building effective partnerships with clients, families, carers and multidisciplinary service providers.
Providing expert clinical advice and advocacy to optimise client outcomes.
Leading outcome measurement and evaluation activities to inform evidence-based practice and continuous service improvement.
Supporting the development of students and less experienced clinicians through education, mentoring and clinical supervision.

✓Experience as a social worker in a health setting

✓Experience providing social work services to older people in a community programme

✓Experience in the training of students and less experienced staff

✓A commitment to person-centred care, clinical excellence and continuous improvement

✓Strong communication, collaboration and stakeholder engagement skills
