Liquid Personnel is recruiting Social Worker for its client's Hospital Discharge Team at Lancashire

Responsibilities

→Complete strengths-based assessments and facilitate safe, timely hospital discharges, ensuring individuals receive the appropriate support and services upon leaving hospital.
→Work collaboratively with multidisciplinary teams, healthcare professionals, service users, and families to develop and implement discharge plans that promote independence and reduce the risk of readmission.
→Manage a diverse caseload, carrying out Care Act assessments, reviews, safeguarding enquiries, and ongoing support planning in line with statutory requirements and best practice.

Requirements

To be successful in this role you must have,

Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
