• Nurse Practitioner
• $145/visit
• Travel: Southwest WA Traveler. Covers Clark, Cowlitz, Lewis (Stevenson, Kelso and Chehalis)
• In-home, local travel
• You choose your hours (min. 24/week). Just assessments, no meds, no Rx, no clinic.

• →Complete 6–9 in-home health assessments/day
• →Review of medical history and risk factors
• →Collect vitals, labs, and screening data
• →Document findings in EMR
• →Educate members on next steps
• →Coordinate follow-up if red flags show
• →Travel across Southwest Washington (Clark, Cowlitz, and Lewis Counties)

• Must have license/certification: APRN, Certified Nurse Practitioner
• Must be registered with Medicare and/or Medicaid
• Valid driver’s license + reliable car
• Able to work independently in the field
