In the  Case Manager Care Coordination position, you will have the opportunity to provide comprehensive care coordination for patients as assigned. This position is accountable for clinical quality of Care Coordination services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care. Case Managers cover units throughout the hospital and generally work in multiple areas each day. Case Managers are responsible for conducting initial assessments on patients, participating in daily multi-disciplinary rounds, communicating the discharge plan and updates to patients, families, and the medical team, as well as delegating tasks to a transitional care associate.
