About the Role:

ComForCare Home Health Care - Decatur is looking for a compassionate and driven RN Case Manager to join our dedicated team in Tucker, GA. This is an exciting opportunity to make a meaningful difference in patients' lives by coordinating high-quality, personalized home health care. Hours: This position is a PRN role that will work around your schedule! Up to three (3) hours per assessment.

Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans for home health patients
Coordinate and manage all aspects of patient care across interdisciplinary team members
Communicate with physicians, therapists, and other healthcare providers to ensure continuity of care
Document clinical findings, care plans, and patient outcomes in compliance with regulatory standards
Educate patients and family members on disease management, medications, and home safety
Ensure compliance with Medicare, Medicaid, and home care agency guidelines

Requirements:

Active Registered Nurse (RN) license in the state of Georgia
Minimum of 1-2 years of clinical nursing experience, preferably in home health or case management
Excellent communication, organizational, and time management skills
Valid driver's license and reliable transportation for home visits in the Tucker, GA area
Proficiency with electronic health records (EHR) and clinical documentation systems
Compassionate, self-motivated, and able to work independently in a field-based setting
