Director of Clinical Services Home Health
Quick Summary
Job Description We are fixing US healthcare by building an AI-native physical care platform. Roughly 40% of what home health spends goes to administrative work, not patient care.
We are fixing US healthcare by building an AI-native physical care platform. Roughly 40% of what home health spends goes to administrative work, not patient care. That cost structure is why agencies have to turn away patients they know they could help. We automate that work, which allows clinical leadership to be free from high-volume, process-intensive back-office functions, with centralized support behind them. Our clinical leaders put their full focus where it matters most: active clinical oversight, skills training and development, and driving the systems and behaviors that keep patients safe and out of the hospital. We have grown significantly over the past year and are seeking someone who wants to join that growth and set the clinical standard for what comes next. The clinical leaders who build our branches today are the people who will lead clinically across regions as we scale. In Houston, we show up for the patients nobody else will, and we own the outcome, not just the visit. If you want the hardest version of this work, with a team that won't let you fail quietly, this is the right place. We want a hungry, low-ego leader who shares credit, pitches in wherever the team needs it, and is good at solving hard problems with other people. Position Overview We are looking for a Director of Clinical Services who can lead clinical execution with high standards and high intensity. The Director of Clinical Services is the senior clinical authority for the branch, owning clinical quality, regulatory compliance, performance management, and clinical execution at scale. You are accountable for outcomes, not just activities. You will partner closely with operations, scheduling, and market leadership to keep care delivery on track, and you will be a key leader in how we scale safely. What You'll Do Own clinical authority and compliance for your branch
Serve as the Home Health Administrator and senior clinical decision-maker for the branch
Own survey readiness year-round rather than in response to a scheduled visit
Ensure the branch exceeds Medicare Conditions of Participation and state regulations
Conduct field supervisory visits and review active cases directly to stay close to patient status
Coordinate next steps after clinical events, ensure care plans are clear and actionable, and hold the standard on documentation quality and timeliness
Build and lead the clinical team
Lead the branch clinical team today and build clinical leadership beneath you as the market grows
Directly manage Branch Clinical Managers and Field Operations Managers
Own clinician development, performance management, and risk mitigation
Address performance issues quickly and fairly, including structured improvement plans when needed
Hire, develop, and lead your branch's clinical staff as census grows
Lead quality, outcomes, and compliance
Own branch clinical quality and outcomes, including rehospitalization prevention
Track HHCAHPS results and quality KPIs, identify trends, and implement coaching or process changes
Partner with our centralized Quality Assurance team on documentation accuracy and OASIS review
Identify recurring clinical and documentation issues and implement process improvements
Be the escalation leader
Serve as the clinical authority when a patient's condition changes or care delivery is at risk
Make calm, decisive escalation decisions for high-risk patients, safety concerns, and incidents
Coordinate with market leadership on sensitive patient situations, hospitalizations, and quality investigations
Who You Are A high-accountability clinical leader who loves developing clinicians and building systems. You bring urgency, clarity, and standards to complex clinical operations. You are energized by coaching clinicians, solving care delivery problems in real time, and holding the line on quality every day. Must-haves

Three or more years of leadership experience at a Medicare-certified skilled home health agency, with clinicians reporting directly to you.
Deep knowledge of Medicare Conditions of Participation and strong clinical judgment
Leadership presence with a strong sense of personal accountability

The ability to hire, develop, and lead your branch's clinical team
A willingness to join a fast moving, growth-oriented team

Experience at an agency with centralized or shared-services back office functions
