Responsibilities

→Complete OASIS comprehensive assessments: Start of Care, Resumption of Care, recertifications, and significant-change visits
→Work with physicians to build and update each patient’s plan of care
→Provide skilled nursing services, treatments, and preventive care within the RN scope of practice
→Monitor patient condition and report changes to the physician
→Teach patients and families how to manage care at home
→Supervise LPNs and home health aides as assigned, including aide supervisory visits
→Document visits accurately and on time in our electronic health record
→Take part in on-call or after-hours coverage as scheduled

Requirements

Active, unrestricted NC RN license (or compact privilege to practice in NC)
At least 1 year of recent clinical nursing experience
Current CPR/BLS certification
Valid NC driver’s license, reliable transportation, and current auto insurance
Ability to manage your own visits, meet documentation deadlines, and use an EHR
Ability to lift up to 50 lbs and assist with patient transfers
Clean background check, TB screening, and no substantiated findings on the NC Health Care Personnel Registry
