Our team provides essential primary care services through a network of community clinics and mobile health units, caring for uninsured and Medicaid populations. • Location: Harrisonburg and Broadway, VA
• Schedule: Monday – Friday 9:30-6:30p. 1-2 Saturdays per month
• Working from Mobile Care RV providing direct primary care services to children and adults in underserved communities at designated locations such as Hope Distributed (Food Pantry & Clothing Ministry) and Navigation Center (Homeless Services Center).
• Partner with City Schools providing sports physicals, immunizations, etc.
• Shared call schedule, with RN first call
• Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically.
• Review quality and outcome metrics monthly with the healthcare team
• Collaborate with local civic and faith-based leaders on community education programs
• Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
• 1+ year APP experience required
• The ability to speak Spanish is extremely helpful in this role, but not required
