The Chesapeake Health Department serves the community within the City of Chesapeake and is seeking a full-time Public Health Nurse to ensure individuals with chronic illnesses, disabilities, or age-related conditions receive safe, coordinated, and person- centered care long term. Face-to-Face assessments are performed on adult and pediatric clients to determine eligibility based upon DMAS standards. This position collaborates with the Department of Social Services (DSS) to conduct joint home visits to assess/screen individuals who may be at imminent risk for nursing home placement or in need of other referral services. Pediatric screenings are performed independent of DSS.

The Public Health Nurse works collaboratively with public health professionals, healthcare providers, community organizations, and other partners to support public health programs and initiatives. This position is critical to protecting vulnerable populations by ensuring access to essential healthcare services.

VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.

In the event of an emergency declaration by the Governor, public health nurses are required to assume other duties for the districts’ emergency preparedness and response (EP&R) activities, including but not limited to points-of-dispensing (POD) mass vaccination events and shelter coverage. During declared emergencies, this will require an adjustment to work hours, location, job duties as well as participation in trainings to meet public health business needs. Additional potential work-schedule variations include participation in community outreach events and other EP&R assignments, such as communicable disease and epidemiological investigation events and locality emergency shelter duties.
