APRN, Palliative Care

Southwestern Vermont Medical Center (SVMC), a member hospital of Dartmouth Health (DH), is seeking an experienced Advanced Practice Provider to join our Palliative Care team.  The ideal candidate will have at least 1-3 years of experience in a palliative care or pain management setting.

• →Provide palliative care consultations and follow up care
• →Conduct comprehensive patient assessments and develop treatment plans
• →Assist with symptom management and pain management strategies
• →Facilitate goals-of-care and advance care planning discussions
• →Collaborate with multidisciplinary teams to optimize patient outcomes
• →Support patients and families through complex medical decision-making
• →Maintain accurate and timely documentation within the electronic medical record

• →Salary range $130,000 - $145,000
• →The position includes a comprehensive benefits package through Dartmouth Health.

• Graduate of an accredited APRN or PA academic program
• National specialty certification as FNP, PNP, Acute or Primary Care A-GNP, WHNP or certification through the National Commission on Certification of Physician Assistants (NCCPA)

• Eligible for RN licensure in the State of VT (APRN only)
• Eligible for APRN or PA licensure in the State of VT
• Eligible for Federal DEA Registration
• BLS certification
