The Sustainable Housing Case Manager – HUD provides case management services to clients currently receiving ACTS Housing Services and community participants who have been provided housing services through ACTS.

✓Conducts comprehensive initial screening process to determine eligibility for program services, providing assessment, intake and enrollment where appropriate.
✓Assist client in developing Individual Service Plan (ISP), assess progress and problem-solving barriers to self- sufficiency regularly.
✓Monitors client’s compliance with Program Agreement.
✓Serves as an advocate and liaison for assigned caseload clients receiving ACTS Housing Services.
✓Provides education and information in building client self-sufficiency around family systems, housing, budgeting, employment, educational training, health and other human services with an emphasis on long-term housing and economic stability.
✓Makes referrals to other services as needed and works with other community partners to provide collaborative services to clients.
✓Executes their assigned program funding within the approved budget and in compliance with funding source requirements.
✓Enters and updates information into the Homeless Management Information System (HMIS), SharePoint and other client databases as required.
✓Maintains client files in accordance with ACTS guidelines and other accrediting agencies for program and grant requirements.
✓Participates in regular supervision with Program Supervisor by providing regular updates and preparing case presentations.
✓Completes required 40 hours of training annually including CPR, First Aid and NARCAN certification and other human service trainings.
✓Promotes community awareness about homelessness and human services.
✓Attends staff meetings, community meetings, training and other agency related activities as required.
✓Conducts ongoing outreach to establish/foster a network of community resources and services.
✓Performs other duties as required.

✓A bachelor’s degree in human services, behavioral sciences or allied field (including criminal justice, education and psychology)
✓AA degree in human services, behavior sciences or allied field (including criminal justice, education and psychology) with 4 years of documented related experience
✓No college degree in human services, behavioral sciences or allied field (including criminal justice, education and psychology) with 5 years of documented related experience.
✓A bachelor’s in human services or behavioral sciences, preferred
✓Prolonged periods of sitting at a desk and working on a computer.
✓Must be able to lift up to 15 pounds at times.
