What We Offer

Shift: 6:45am to 7:00 pm with every other weekend.
Location: Covering Orange, Osceola and Seminole Counties.
- Provides skilled nursing care to hospice patients requiring crisis care in various settings.
- Assesses patient conditions and symptoms necessitating crisis care.
- Administers medications and treatments as prescribed.
- Monitors and manages symptoms to ensure patient comfort.
- Communicates with the healthcare team regarding patient status and care plans.
- Educates and supports family members and caregivers.
- Documents patient care and updates medical records accurately.
- Coordinates with other healthcare providers to deliver comprehensive care.
- Responds promptly to patient needs and emergencies.
- Maintains compliance with state and federal regulatory standards.
- Participates in interdisciplinary team meetings and care planning.
- Performs other duties as assigned.
Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee.
Knowledge of electronic medical record.
Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form.
Ability to follow standard precaution guidelines for infection control, including when providing patient care.
- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
- N/A
- 2+ experience as a registered nurse [Required]
- Home health and/or acute care nursing experience [Preferred]
- N/A
- Registered Nurse (RN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]
- Driver's License (DL) [Required]
- Certified Hospice and Palliative Nurse (CHPN) [Preferred]
