What We Offer
~1 min readConduct comprehensive patient assessments to determine physical, mental, social, and spiritual needs related to wound, ostomy, and continence care.
Develop and document individualized care plans based on patient assessments, selecting appropriate interventions and supplies.
Maintain accurate and timely patient documentation in the electronic medical record.
Administer treatments and medications safely while monitoring patient response and adjusting care plans as appropriate.
Provide direct wound, ostomy, and continence care that promotes healing, patient comfort, and optimal outcomes.
Educate patients and families on wound care, ostomy management, continence strategies, self-care techniques, and lifestyle modifications.
Provide education and clinical guidance to nursing staff and other healthcare professionals on wound, ostomy, and continence care.
Collaborate with physicians, nurses, and interdisciplinary team members to coordinate comprehensive, patient-centered care.
Monitor, evaluate, and document patient progress, revising care plans based on patient response and clinical outcomes.
Respond to urgent patient care needs and participate in on-call coverage, including weekends and holidays, as required.
Assess the educational needs of nursing staff and support competency in skin integrity, wound prevention, and specialty care.
Incorporate evidence-based practice, current research, and clinical guidelines into patient care and quality improvement initiatives.
Communicate patient care concerns, barriers, and recommendations to the Clinical Nurse Manager and interdisciplinary team to support safe, effective, and coordinated care.
Schedule: Full Time, 40 hours/week
Shift:
Day shift 8:00 AM to 4:30 PM
Start / Stop times may vary depending on need.
Required Skills and Qualifications:
Current knowledge of wound, ostomy, and continence care principles and best practices.
Ability to communicate effectively with patients, families, physicians, and interdisciplinary team members.
Ability to provide patient-centered care while working effectively with individuals from diverse backgrounds.
Ability to prioritize tasks, manage multiple responsibilities, and meet deadlines in a fast-paced environment.
Strong organizational and time management skills.
Strong collaboration and teamwork skills.
Ability to provide compassionate support to patients and families while maintaining professional boundaries.
Knowledge of infection prevention and control practices, medication administration, storage procedures, and applicable regulatory requirements.
Commitment to maintaining a safe, clean, and compliant work environment.
Reliable attendance and the ability to work scheduled hours, including schedule adjustments based on patient care or clinic needs.
Preferred Skills:
Proficiency with Microsoft Office applications.
Commitment to ongoing professional development and continuing education.
Strong verbal and written communication skills.
Ability to manage competing priorities and navigate challenging situations professionally.
Knowledge of outpatient hospital services.
Experience collaborating with physicians and interdisciplinary healthcare teams.
Education:
Bachelor of Science in Nursing (BSN). (Preferred)

Advanced Practice Registered Nurse (APRN). (Preferred)
