Anderson Health Services is seeking a Care Coordinator / Case Manager to support adolescents with mental health and behavioral health needs, including those with a history of complex trauma. This role focuses on coordinating services, facilitating communication across care teams, and ensuring high-quality, trauma-informed care in a structured treatment environment.
The ideal candidate is organized, relationship-driven, and experienced in youth services, care coordination, and family engagement.
Responsibilities

Coordinate and facilitate Child and Family Team (CFT) meetings

Lead development and implementation of individualized service plans (ISPs)
- →
Provide case management and care coordination for youth and families
- →
Build strong relationships with clients, families, and multidisciplinary teams
- →
Support identification of needs, goals, and outcomes using a person-centered approach
- →
Engage and incorporate natural supports and community resources
- →
Promote youth and family ownership of the Wraparound process
- →
Identify strengths and integrate them into treatment planning
- →
Address barriers to participation and implement solutions
- →
Maintain accurate clinical documentation, progress notes, and reports
- →
Ensure compliance with HIPAA, confidentiality, and regulatory standards

Educate families and providers on program services and expectations
- →
Collaborate with clinicians, direct care staff, and supervisors
- →
Support a safe, structured, and therapeutic environment
Requirements

Bachelor’s degree in Human Services, Social Work, Psychology, or related field (Master’s preferred)

Minimum 2 years of experience in mental health, case management, or youth services

Strong skills in care coordination, treatment planning, and team collaboration

Experience working with adolescents, families, and trauma-informed models preferred

Ability to manage multiple cases in a fast-paced environment

Standing, walking, bending, and climbing stairs

Ability to respond to behavioral crises and safety situations
-
May involve physical intervention when necessary to maintain safety
-
Ability to work with individuals who may display aggressive or challenging behaviors

Must be 21 years or older

Valid North Carolina driver’s license with good driving record

Demonstrates effective emotional regulation and professionalism

Must pass background check and drug screening
