The Inpatient RN Case Manager works with the interdisciplinary team to coordinate the care of the patients focusing on what the patient will need post hospitalization. They will assess, evaluate and coordinate a safe discharge plan with the patients/families’ goals of care as their guide. The goal of the RN case manager is to transition the patients to the next level of care in a timely and safe way with the wishes of the patients/families at the forefront of those transitions. This professional is responsible for maintaining very effective inter-professional collaboration across the clinical teams involved in patient care, as central to the success of the work.

Requirements

  • Bachelor's degree in nursing or equivalent experience.
  • Registered nurse license in the Commonwealth of Virginia or from a state that is part of a Compact agreement with Virginia
  • Minimum of 2 years in an acute hospital setting
  • BS or MS in Nursing
  • National certification in case management
  • 2-3 years of recent experience in discharge planning in an acute care setting preferred
  • Ability to work independently with a minimum of direction, anticipate and organize workflow, prioritize and follow through on responsibilities.
  • Possesses strong clinical knowledge/expertise of common diagnoses, current treatments, and current physical and psychosocial sequelae.
  • Excellent verbal and written communication skills and the ability to work with diverse populations.
  • Collaborates with Social Worker on patients with complex psychosocial needs and complex family dynamics.
  • Knowledge of long-term care resources and qualifying requirements; Medicare, Medicaid, third party payors; financial, transportation, pharmaceutical assistance and other community and health systems resources for assisting with patient needs.
  • Able to identify and report quality-of-care and risk management issues to the Director in a timely fashion.
  • Maintains familiarity with all hospital and department specific policies and procedures.
