Under the direction of licensed medical and/or licensed nursing personnel, the Licensed Vocational Nurse (LVN) will support an interdisciplinary team of clinicians in delivering coordinated, patient centered care and will provide direct nursing care as needed. This role is responsible for providing comprehensive telephonic and clinic-based support for clinical teams and may assist across multiple service lines based on organizational needs and team capacity. The LVN practices vocational nursing in accordance with the Texas Nursing Practice Act and rules of the Texas Board of Nursing and by adhering to organizational policies, procedures, and guidelines. Responsibilities

  • Provide clinical care coordination, and administrative support to interdisciplinary care teams under RN/provider direction.
  • Serve as a liaison and point of contact between patients, caregivers, providers, and external partners (including hospital staff, home health, and other care teams) to facilitate ongoing clinical care coordination. Responsible for sending, receiving, and managing clinical communications across care settings, ensuring timely follow-up and continuity of care.
  • Assist with medication management by identifying potential barriers, responding to patient questions about side effects and drug interactions, coordinating prescription refills and prior authorizations.
  • Reinforce education and escalating medication-related concerns (e.g., side effects, interactions) to RN/provider as appropriate.
  • Follow up on patients in acute care settings and identify those who may benefit from additional services (e.g., hospice), escalating to the RN/provider for evaluation and referral as appropriate.
  • Support referral management by pre-screening referrals for appropriateness for specialty related programs. Initiate referrals within scope, authorizations, and service requests as directed; verify completion and follow-up.
  • Assist patients and families with navigating care processes and understanding plan of care and follow up steps.
  • Support transitions of care and follow-up coordination for patients in acute or post-acute settings.
  • Recognize and perform basic assessment of patient symptoms and clinical concerns and escalate both routine and urgent/abnormal findings to RN/provider per protocol.
  • Document patient contacts, communications, and delegated nursing activities in a timely manner in EHR.
  • Assists with durable medical equipment (DME) ordering, management, and follow up process to ensure patients receive their durable medical equipment in a timely manner, understand proper use, and schedule necessary follow up visits/care related to the DME item.
  • Promote patient’s knowledge of medical conditions and improve self-management practices.
  • Contribute to care plan execution by reinforcing RN/provider-developed plans, tracking patient progress, and identifying barriers to care.
  • To improve the patient experience and to avoid delays in care, as needed will support provider teams by assisting with completing pre-visit planning (PVP), in-basket management, rooming and wrap up, and clinical duties following established protocols, competency validation, and in accordance with Standing Delegation Orders.
  • As directed, may support residential/home nursing visits to supplement enhance periodic provider visits.
  • Participates in daily unit huddles, clinic meetings, patient conferences, and education sessions—both in person and virtually—as required to support patient care and quality initiatives.
  • Performs other duties as assigned.

 

  • Knowledge of palliative and supportive care principles.
  • Knowledge of chronic and life-limiting conditions and their progression.
  • Effective communication skills for interacting with patients, families, and interdisciplinary teams with empathy and clarity.
  • Care coordination skills, supporting care plans, follow-ups, and connections to resources across the continuum.
  • Ability to provide compassionate, patient- and family-centered care, including emotional support during serious illness and end-of-life situations.
  • Ability to work collaboratively within an interdisciplinary team, while maintaining professionalism, cultural sensitivity, and respect for patient preferences and goals of care.
  • Knowledge of ambulatory care practices and clinic workflows, including patient intake, vital signs, documentation, and basic clinical procedures.
  • Ability to manage multiple priorities in a fast-paced clinic environment, maintaining accuracy, efficiency, and a high level of patient service.
