Responsibilities

Education: Associate Degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred.

Experience: 5+ years of experience in healthcare, geriatrics, home health/home care, hospice, case management, or social services preferred.

Relationship & Communication Skills: Exceptional bedside manner, active listening, de-escalation, and conflict-resolution abilities.

→

Tech Savvy: Comfortable utilizing electronic health records / home-care management software (experience with ClearCare/WellSky is a plus) and mobile documentation.

→

Mobility & Independence: Valid driver’s license, reliable personal vehicle, and active auto insurance for local travel throughout New York area.
