**Job Summary:**
The Community Health Center of Niagara (CHCN) is seeking an experienced Licensed Practical Nurse (LPN) to join our team. The LPN will deliver nursing care under the supervision of the Registered Nurse and/or Physician. **Responsibilities:**

**General:**
- Adheres to agency policies and procedures regarding confidentiality.
- Participates in quality improvement activities.
- Communicates effectively with patients and staff.
- Maintains a professional demeanor when interacting with patients and staff.
- Adheres to infection control principles.
- Participates in Quality Improvement and Quality Assurance activities as needed.

**Specific:**
- Maintains a clean, safe, and orderly work environment.
- Performs supportive duties to ensure efficient patient flow.
- Prepares patients for examinations and procedures.
- Prepares examination and procedure rooms.
- Assists providers during examinations and procedures.
- Performs routine diagnostic tests.
- Collects and processes specimens.
- Prepares and administers medications as directed by the healthcare provider.
- Reviews lab results and reports abnormal findings to the provider.
- Maintains equipment according to agency policy.
- Ensures the integrity of the medical record.
- Maintains documentation in logs as required.
- Provides patient education.
- Administers immunizations for adults and children.
- Recognizes emergency situations and notifies appropriate personnel.
- Requires regular attendance and participation in meetings.
- Performs other relevant duties as assigned.

**Physical Demands:**
Requires periods of sitting, standing, walking, bending, and stooping. Evening work may be required.

**Salary Status:**
Hourly; non-exempt.

**Minimum Qualifications:**
- Completion of an accredited Licensed Practical Nurse Program.
- Licensed as a Practical Nurse in the state of New York.
- Three years of experience in a healthcare setting.
- Computer literacy.
- Proficiency in Microsoft Word, Excel, and Outlook.

**Preferred Qualifications:**
- Completion of an accredited Licensed Practical Nurse Program.
- Licensed as a Practical Nurse in the state of New York.
- Five years of experience in an ambulatory healthcare setting.
- Computer literacy.
- Proficiency in Microsoft Word, Excel, and Outlook.

This is a union position with 1199SEIU.

Hourly Rate: $22.78 - $27.51.
