**Job Summary:**
As a Mental Health Counselor at Community Health Center of Buffalo, Inc., you will provide outpatient clinical services in an Article 31 setting, assisting individuals facing mental health and co-occurring challenges. This role involves delivering relationship-based clinical work within an interdisciplinary team environment, aiding patients in accessing care, establishing stability, and progressing in their recovery through trauma-informed, person-centered methods.

**Responsibilities:**
- Conduct comprehensive psychosocial and diagnostic assessments, including risk evaluations.
- Develop and implement individualized, recovery-oriented treatment plans.
- Provide individual therapy utilizing evidence-based approaches like CBT, DBT, and trauma-informed care.
- Administer crisis intervention and coordinate higher levels of care as necessary.
- Collaborate with medical providers, care teams, and community partners to facilitate whole-person care.
- Participate in team meetings, case conferences, and clinical supervision.
- Connect patients with community resources such as housing, substance use treatment, and social services.
- Maintain timely and compliant clinical documentation following OMH and Article 31 standards.
- Meet productivity expectations while delivering quality, patient-centered care.

**Qualifications:**
- LMHC, LMSW, or LCSW (NYS permit holders encouraged to apply).
- 3+ years of clinical experience preferred.
- Experience in trauma-informed care; EMDR experience considered advantageous.
- Strong interpersonal, communication, and organizational skills.
- Ability to work effectively in a fast-paced, team-based environment.

**Schedule & Physical Requirements:**
- Combination of sitting, standing, and movement throughout the day.
- Some evening or weekend availability may be required.

**Compensation:**
- $52,000–$58,000 annually.
