Responsibilities

- → Conduct intake meetings and ongoing check-ins to assess needs and goals
- → Connect families to appropriate community resources and support services
- → Coordinate communication between families, providers, and agencies
- → Assist families in completing required documentation and applications
- → Maintain accurate and timely records in accordance with agency standards
- → Monitor service plans and follow up to ensure needs are being met
- → Advocate for families within school, healthcare, and social service systems
Requirements
