Responsibilities

→Provide comprehensive outpatient care for adult and geriatric patients
→Conduct preventive screenings, chronic disease management, and acute care visits
→Develop evidence-based, individualized care plans
→Maintain accurate, timely documentation in the EMR
→Participate in care coordination, daily huddles, and practice-wide quality improvement initiatives

Requirements

Board-Certified or Board-Eligible in Family Medicine or Internal Medicine
Arizona medical license (or eligibility to obtain)
