The Telephonic Nurse Care Manager provides remote, patient-centered nursing support to individuals living with chronic and complex health conditions, including heart failure, COPD, diabetes, and hypertension. Through virtual assessments, remote patient monitoring, and proactive outreach, this role helps patients manage their health safely and effectively from home. You will interpret biometric and interactive voice response (IVR) data, identify changes in clinical status, and develop individualized, evidence-based care plans. Working closely with patients, caregivers, and interdisciplinary care teams, you will promote self-management, coordinate care transitions, and help prevent avoidable hospitalizations and emergency visits. This position combines clinical judgment and compassionate communication with technology-enabled care delivery in a collaborative, fast-paced virtual environment. It is fully remote and offers the opportunity to contribute to improved patient outcomes and more accessible healthcare.

  • Conduct comprehensive telephonic and virtual nursing assessments, reviewing patient health status and interpreting biometric, remote patient monitoring (RPM), and IVR data to identify trends and potential concerns.

  • Develop, implement, and regularly adjust individualized nursing care plans based on patient needs, personal goals, evidence-based practices, and established clinical protocols.

  • Use Motivational Interviewing techniques to encourage healthy behavior changes, treatment adherence, and greater patient engagement in self-management.

  • Identify clinical risks, changes in patient condition, and urgent situations, escalating concerns promptly in accordance with clinical guidelines.

  • Coordinate care with internal and external interdisciplinary teams, including arranging referrals and supporting transitions across the care continuum.

  • Deliver proactive nursing interventions designed to improve clinical outcomes, reduce preventable hospitalizations and emergency department visits, and lower overall care costs.

  • Maintain accurate, timely, and comprehensive documentation of all patient interactions and care management activities in the electronic health record (EHR).

  • Participate in quality improvement initiatives, chart audits, and performance reviews to strengthen clinical quality and patient outcomes.

  • Complete care management activities in accordance with client-specific workflows, performance expectations, and service-level requirements.

  • Maintain compliance with applicable clinical, regulatory, HIPAA, and organizational standards.
