Liquid Personnel is recruiting Social Worker for its client's Adult Services Team at Rhondda Cynon Taf.

We are recruiting for a Social Worker to join a busy Adult Services team based at Cynon Principal Offices. You'll be supporting adults aged 18+ with complex and long-term care and support needs, helping individuals achieve positive outcomes and maintain independence within their communities.

Responsibilities

→Manage a complex Adult Services caseload
→Complete assessments, reviews and support plans
→Support individuals with physical and mental health needs
→Promote positive, person-centred outcomes
→Work collaboratively with partner agencies and professionals

To be successful in this role you must have,

Registered with Social Care Wales
Previous Adult Services experience
Strong assessment and caseload management skills
Passion for delivering high-quality support
