Liquid Personnel is seeking an experienced and dedicated Adults Social Worker to join our client’s Countywide Reviewing Team, based in East Riding of Yorkshire.

Responsibilities

In this role, you will:

→Conduct proactive reviews of care arrangements: Assess residents in care homes with 1:1 support and individuals receiving Direct Payments to ensure packages are appropriate, least restrictive, and proportionate.
→Update assessments and care/support plans: Visit individuals in their home environment, review their needs, and revise documentation to reflect current circumstances.
→Submit funding requests and liaise with providers: Work collaboratively with care providers to adjust support levels, reduce unnecessary restrictions, and ensure funding applications are accurate and timely.

Requirements

To be successful in this role you must have:

Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
