Social Worker - Hospital Discharge Team

Liquid Personnel is recruiting Social Worker for its client's Hospital Discharge Team at Shropshire.

This is an excellent opportunity for a dedicated practitioner to support adults with complex care and support needs through safe, timely, and person-centred discharge planning from acute hospital settings.

The successful candidate will work within a fast-paced multidisciplinary environment, supporting individuals and their carers through periods of transition whilst promoting independence, wellbeing, and positive outcomes. You will work closely with health professionals, families, care providers, and community services to ensure people receive the right support at the right time and can safely return to the most appropriate setting following hospital admission.

This role requires a confident Social Worker with strong assessment skills, knowledge of Adult Social Care legislation, and experience of working within hospital discharge pathways.

• →Undertaking Care Act 2014 Assessments to identify eligible care and support needs and inform discharge planning.
• →Supporting safe and timely hospital discharges for adults with complex health and social care needs.
• →Completing Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
• →Arranging Pathway 3 placements and longer-term care arrangements where ongoing support and residential care are required.
• →Working collaboratively with individuals, carers, families, health professionals, and partner agencies to coordinate effective discharge plans.
• →Responding to referrals, undertaking assessments, and prioritising urgent discharge activity within a busy hospital environment.
• →Developing person-centred Care and Support Plans that promote independence, recovery, and positive outcomes.
• →Assessing and managing risk appropriately to ensure safe discharge arrangements are in place.
• →Coordinating community-based services, care packages, equipment, and support arrangements to facilitate successful discharge outcomes.
• →Promoting strengths-based practice and supporting individuals to maximise independence and wellbeing following discharge.
• →Participating in multidisciplinary meetings, discharge planning discussions, and case reviews.
• →Maintaining accurate, timely, and legally compliant assessments, reports, case records, and support plans.

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
