Sentara Health is seeking to hire a qualified individual to join our team as a Social Worker.

The Social Worker BSW - Home Health performs a variety of casework and provides social work services to patients, families, and significant Must develop and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Part-Time (24 hours/week)

Day Shift (8AM-5PM)

Sentara Hospice Services - South Boston, VA (travel can include up to a 50-mile radius for position)

Bachelor's Level Degree in Social Work (Required)

Valid Driver's License (Required)
BLS (Required within 90 days of hire)

1 year of social work experience (Required)
Previous Home Care/Hospice experience (Preferred)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $64,625.60 - $100,256.00 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
