Job Description

Job Title: Float-Team Supervisor/Health Coordinator Department:

Community Living
Reports To: VP of Community Living Supervises:

Direct Support Professionals FLSA: Non-Exempt


SUMMARY
A Float Team Supervisor/Health Coordinator is a critical team member in the Community Living Department. Their primary responsibilities include being familiar with LU’s standard on the day to day functions to successfully managing the operation of an ISL and/or Group Home setting, providing on going training and support to new Team Supervisors and working closely with the Intake Team to coordinate the staffing needs for new individuals served moving into the Community Living Department. The location and/or individuals served assigned to the Float Team Supervisor/Health Coordinator will fluctuate and is based on the needs of the department. However, the Float Team Supervisor/Health Coordinator will be responsible for covering any TS/HC vacancies and maintaining and/or improving the day to day operations of the home until a new Team Supervisor and/or Team Supervisor/Health Coordinator is hired and trained to ensure a smooth transition.


BASIC FUNCTIONS AND GENERAL RESPONSIBILITIES:

Establish and maintain professional working relationships with all stakeholders.
A. Develop a positive working relationship with the CL individuals served, guardians, and Service Coordinators
B. Maintain a working relationship with the CL Team Supervisors/Health Coordinators, Health Coordinators, Program Managers and Directors as well as other departments and programs within LU.
Collaborates with Community Living Team Supervisor(s) and Program Manager(s) to develop action plans that focus on improving the quality and efficiency of services provided to the individuals served.
A. Responsible for stepping in and acting as Team Supervisor/Health Coordinator when there are TS and/or TSHC vacancies (refer to TS and TSHC Job Description).
Responsible for delivering services at the highest quality while utilizing best practice.
A. Provides support to the individuals served in making informed choices, growing in self-esteem and personal dignity, and achieving community integration and participation.
(a) Be knowledgeable of the individuals served Rights and Responsibilities
(b) Ensure services being delivered are person-centered and meaningful to the individual served.
(c) Be an active participant in PCSP/ISP meetungs and rec
• Ensures documentation is compliant will all applicable laws, rules and regulations.
A. Ensure confidential and health related records are kept secure.
(a) Electronic records and health and safety records are complete at the beginning and end of each staff schedule.
(b) Ensure General Event Reports are completed according to agency policy and rules and regulations of all regulatory/governing bodies.
(c) Ensure all Medication Administration Records, physician orders, and other medical forms are current.
(d) Ensure tracking system for medical appointments, follow-up appointments and routine lab work is current and accurate, including Therap calendar.
(e) Displays mastery in completing TS Weekly Checklists.
• Knowledgeable all health and safety guidelines, rules, policy and procedures.
A. Ensure the health and safety of all individuals through teaching and providing support in areas of personal care needs, dental hygiene and other health-related RN delegated duties.
B. Contribute to a home-like, safe, sanitary environment for person-centered living. Provide feedback and follow up to ensure supplies are available and environment is maintained to highest standards.
(a) Ensures hazardous and potentially harmful materials locked and out of individuals’ reach unless the individual has demonstrated safe handling skills.
(b) Independent in observing and teaching the guidelines for Emergency Drills and real emergencies.
(c) Familiar with supporting individuals served with scheduling, arranging transportation and/or participating in medical appointments.
(i) Ensure team members are supporting individuals as directed in their PCSP/ISP and recommended by their physicians
(ii) Fluent in providing professional, timely and clear correspondence to physicians, RNs, guardians, and support staff to keep team members informed of any health care changes.


GROWTH- Lead program growth, expansion, and improved efficiencies
• Meet strategic planning goals, objectives, and outcome measures for the caseload under their supervision.
• Collaborate with Sr. VP of Community Living amd members of the Training Department to identify training opportunities and professional development for new Team Supervisors and DSPs.
C. The ability to effectively and efficiently use computers or mobile devices to input, access, modify, or output information. This includes the ability to quickly access, store and retrieve data.
• Works closely with the Program Manager Float, Sr. Program Director, and Intake Team regarding new locations preparing to open.
A. Works with the Intake Team on site development and roommate matching to ensure a smooth transition to community living services.
B. Work with and train new team members (Team Supervisor/Health Coordinators included) on the basic functions of the home, which includes but is not limited to individuals’ goals, specific needs for the individuals served, guardians’ goals for their individuals, expectations for the home.
i. Ensure staff is trained and home is prepared for transition within 90 days of opening.


FINANCIAL STRENGHT- Makes strong financial decisions.
• Exhibits strong skills when advocating for new team members to reduce or eliminiate overtime at locations assigned to.
A. Work closely with the CL Leadership and Recruiting Team regarding ways to reduce OT
B. Work closely with the CL Leadership and Training Department regarding training opportunities for staff retention in high turnover locations.
• Must be independent with maintaining the agency and individuals funds assigned to work-site location.
A. Ensure all funds are safely stored and cash logs are available and maintained according to agency policy.
B. Exhibits mastery in reconciling all finance accounts and ensure funds are accounted for according to agency policy.
1. Provides weekly (at minimum) oversight on all finance accounts/ledgers and ensures funds are accounted for according to agency policy.
C. Ensure individuals and Direct Support Professionals have the necessary funds to participate in chosen activities.
• Responsible for independently completing payroll weekly (Sunday mornings) and ensuring team members are paid correctly to avoid under or over payments.
A. Ability to train new team members on Workforce and payroll processing.


TEAM BUILDING & SUPERVISION- Ensure all programs are adequately staffed and all team members are well trained and supported in their position.
• Provider 24-hour on-call support to new Team Supervisor and or Direct Support Professioinal(s) to assist with addressing individual and staffing emergencies. When unable to provide this support due to PTO, ensure team is notified of the temporary coverage providing this support.
A. Communicate any change in functioning, health status, or behavior to members of the program team, including caseloads director
• Provides regular and timely communication/feedback.
A. Conducts one (1) house and one (1) staff meeting monthly with all individuals served living and team members working at their assigned location.
B. Provide regular and consistent communication with Program Managers and members of administration to promote professional collaboration between departments.
C. Provides regular health updates to the RN and/or Program Manager and seeks clarification when necessary.
• Ensure all Direct Support Professionals understand how to implement each individuals served PCSP/ISP and provides services in accordance with each individual served.
A. Ensure team members are confident of the spiritual and emotional needs, socialization needs, and all personal care needs for each individual served are provided according to best practice.
• Provide training to newly retained DSPs and retraining to seasoned DSPs (as needed) assigned to the assigned work-site location.
A. Provide timely feedback to DSPs in the areas of performance, processes, produres, and scheduling needs.
1. Publish Workforce Schedules 1 month in advance to alert DSPs of their schedule to avoid any staff shortages or conflicts.
2. Ensure staff vacancies and shortages are covered even if it requires the Float Team Supervisor/Health Coordinator to fill the shift
3. Responsible for ensuring staff are scheduled for less than 60 hours per week. Receive prior approval from homes leadership team for any team members scheduled over 60 hours per week.
4. Recommend corrective actions, up to and including termination for DSPs assigned to work-site location.
•All other duties as assigned.


COMMUNICATION – Foster strong communication within and between all programs and the overall organization.
• Ensure information is relayed throughout all levels of programs in a way that builds team, and all team members receive the information needed.
• Establish and maintain close working relationship with all stakeholders, including families/guardians, funders, and referral agencies, and solicit regular feedback for quality improvement.
• Responsible for timely and ongoing communication with Life Unlimited Inc. teams.
• Interact in a professional manner with agency staff and internal/external stakeholders, funding/referral agency representatives to promote high quality care for all individuals.
• Maintains open and consistent communication with individuals, families/guardians, other health-care providers and regulatory oversight agencies (DMH, DHSS, etc.).

Physical Expectations
• This position requires extended periods of walking and standing with some periods of sitting, which is dependent up the needs and activities of the individuals supported. It may require light to heavy lifting, up to 50 pounds independently, and occasional stooping, kneeling, bending, and/or climbing stairs depending on the needs of the individuals supported.
• The position will require occasional physical intervention if confronted by a person displaying combative or potentially dangerous behavior.
• Driving an agency or personal vehicle is a part of community integration for the individuals we support.


Working Conditions
Office/home environment with noises from appliances, telephones, and occasional small machinery (lawn mower, etc.). May be exposed to bloodborne pathogens and/or infectious diseases. Occasionally, prolonged hours or unusual chores to meet the unexpected needs of the individuals (i.e. medical and/or behavioral).
QUALIFICATIONS
• Must have a high school diploma or General Equivalency Diploma (GED).
• Must have at least three (3) years of successful employment in a work environment similar to that described in this job description with one (1) year being in a supervisory position.
• Must possess good communication skills both orally and in written form.
• Must have ability to use computer and other electronic equipment.
• Must have a desire to manage a team of staff supporting individuals with developmental disabilities and advocate for them in any possible way.
• Must possess self-motivation, initiative and independence to work without direct supervision.
• Must be flexible, reliable and dependable. Must have the ability to plan and prioritize meeting deadlines.
• Must be able to manage and embrace change, look for ways to improve own performance and accept feedback.
• Must obtain and maintain training and certification in all required areas such as Abuse/Neglect, CPR, First Aid, Level 1 Medication Administration, Positive Behavior Supports, MANDT Technical Skills and Crisis Intervention.
• Must have a reliable personal vehicle for transporting individuals served.
• Must have or obtain (during team member orientation) a valid Class E driver’s license - must carry minimum coverage applicable by law for auto insurance
